Animal Hospital (Anomaly) tells most of its story through recurring characters at the check-in window rather than cutscenes. This page collects what's actually documented about each one, sourced from the Animal Hospital Wiki and checked July 2026. Where a detail is the wiki's own theory rather than a confirmed fact — like Rattina's identity — that's marked clearly rather than presented as settled.

Dr. Harlow, Your Supervisor

Dr. Harlow is the doctor who runs the hospital and grades the player's performance. He appears at the end of each of the first six shifts to explain new mechanics and unlock new areas — shutters and cameras after Shift 1, the Supplies Shop and Emergency Wing after Shift 2, the Taser after Shift 3. Shift 6 is his last scripted appearance, where he warns that 3 animal deaths in a run count as a failure condition. After that he stops proactively approaching the player but remains findable in the lobby for extra dialogue — an anomaly version of him can even be spotted in one of the lobby's exterior windows as an Easter egg.

Barney: The Serial Killer Hiding in Plain Sight

Barney is a recurring "Returning Character" who visits the check-in window repeatedly across Shifts 2-7, each visit escalating: first he asks for coffee, then for his red briefcase to be kept safe, then for his photo to be taken (the first hint he isn't what he seems), then for a scalpel, and finally to be hidden from the police. He's revealed to be a serial killer who used a kitchen knife to incapacitate Officer Duckman. Refusing him at any point (closing the shutters, or not giving coffee in time) costs you his rewards but carries no other penalty — he only threatens, never follows through, and simply stops returning.

Officer Duckman: The Investigation

Officer Duckman is the police officer investigating Barney. He only shows up once the player has agreed to hide Barney behind the front desk on Barney's final visit, and never appears at all if Barney was refused earlier. Turning Barney in gets Duckman attacked and fainted — he needs immediate treatment to survive, and gives no reward either way. Refusing to turn Barney in sends Duckman away asking the player to report suspicious activity instead.

Liz: A Fired Journalist's Investigation

Liz — full name Lisbeth, a caracal — formerly worked for the Animal Corporation's news channel and was fired for telling the truth about the anomaly infection. Across Shifts 10-18, she hands the player her camera and asks for photos of a specific patient across four visits, each photo revealing a deeper stage of the infection. The third photo rewards a Chocolate Bar; the fourth turns the photographed patient into a Mimic or Shapeshifter that attacks. Photographing the wrong patient at any point causes Liz to reclaim her camera permanently.

Ratthew and Rattina: Twins, and a Government Cover-Up

Ratthew is a paranoid but kind-hearted rat character who reveals lore gradually across several visits during Shifts 2-4: he hides his identity at first, then warns the player to trust no one, then reveals that people mistakenly think he's an anomaly, and finally says he must disappear because "the government" is after him. In his last appearance he reveals he had a twin sister, Rattina, who worked for the Animal Corporation and vanished — and hands the player a key and a note before leaving for good. The key opens a safe behind a cat poster in Room 5 containing a photo of Rattina, a "TOP SECRET" suitcase, and a note referencing an experiment involving something called "the Shadow."

Rattina herself is never directly encountered in-game — everything known about her comes from that photo and Ratthew's dialogue. The wiki itself frames her connection to another in-game entity, the Stalker, as theorized and unconfirmed, based only on hints in Ratthew's dialogue rather than a stated fact. This page presents that exactly as hedged as the wiki does — not as a confirmed twist.

Ron from Accounting: The Hospital's Institutional Memory

Ron from Accounting is a blue rabbit NPC who shows up as a patient from Shift 2-3 onward and is unusually knowledgeable about the hospital's dangers — he's personally encountered the Bed Monster, Mass of Eyes, and Stalker, and knows hospital history most patients wouldn't, including that the previous Intern was eaten by the Bed Monster and that patients are described as "infected" rather than simply sick. He always carries Maple Syrup, which happens to be one of the two known ways to make the Bed Monster leave a patient alone.

Background Cast: News Bunny and Head Banger

Not every piece of Animal Hospital Roblox lore comes from a full character arc. The News Bunny is a minor green rabbit reporter who appears on the check-in TV starting Shift 4, right before the first Ambulance Event, giving the broadcast its "breaking news" framing. Head Banger, despite functioning mechanically as an enemy encounter between Shifts 2-5, adds to the hospital's unsettling atmosphere simply through its behavior — banging its hollow-eyed head against the check-in glass while the music cuts out, blocking the shutters from closing until it eventually leaves on its own.

What's Still Unconfirmed
